Glucosamine Supplementation May Be Associated with Worse Outcomes in Alzheimer's Disease

Abnormal glycan metabolism in the central nervous system has been implicated in the development of several neurodegenerative disorders. In Alzheimer's disease, altered glycan processing can influence the post-translational modification of tau protein, a key contributor to disease progression. Because glucosamine is an important substrate in glycan metabolism and is widely used as a dietary supplement for joint health, researchers conducted a study to investigate whether glucosamine supplementation affects outcomes in patients with Alzheimer's disease.

The retrospective study analyzed the health records of approximately 50,000 patients with Alzheimer's disease–related dementia treated within the University of Florida Health system. Around 8% of these patients were classified as glucosamine users, defined as individuals who had taken glucosamine continuously for at least one year after their dementia diagnosis.
Compared with non-users, patients with Alzheimer's disease who used glucosamine had a 25% higher risk of death within 10 years. In addition, glucosamine use was associated with more rapid disease progression and a more severe clinical course. Interestingly, no significant difference in outcomes was observed among patients with mild cognitive impairment, suggesting that the association may be specific to established Alzheimer's disease rather than earlier stages of cognitive decline. Although this observational study cannot establish a causal relationship, the findings raise concerns regarding glucosamine use in patients with Alzheimer's disease. The researchers emphasized the need for prospective clinical trials to determine whether glucosamine directly influences cognitive decline and disease progression in individuals with dementia.
